Case Manager Team Lead

Job Description

A day in the life of a Case Manager Team Lead involves coordinating administrative tasks, advocating for individuals with developmental disabilities, and mentoring staff members. You will collaborate with a team of 6-10 SSAs and dedicate time to guiding new and tenured employees in all aspects of the SSA role.

In the Team Lead role, you will maintain a modified caseload to remain available to support team members. You will assist with managing crises and serve as an integral member of the On-Call team. Operating in various settings, including the community, the agency office, and a home office, you will actively contribute to continuous improvement and significantly maintain a positive work environment for the SSA team.

Hybrid – remote and at HCDDS Kingsley Center: 5093 Kingsley Drive, Cincinnati, OH 45227.


Job Responsibilities

  • Collaborate with the SSA Supervisor to assign individuals to SSA caseloads and review My Plans.
  • Provide caseload coverage in vacancy or absence cases while mentoring new SSA staff.
  • Assist in tracking data elements for each person on the assigned team’s caseloads.
  • Maintain a caseload that does not exceed half the typical size for the assigned team.
  • Facilitate the development of single service plans for individuals and families.
  • Act as the primary point of contact and aid individuals and families in identifying, selecting, and obtaining/maintaining services, support, and benefits.
  • Ensure timely and accurate completion of all required documentation with supervisory support.
  • Monitor individual plans for compliance and quality in service delivery.
  • Travel to various county locations for safe and efficient service planning and monitoring in line with agency procedures.
  • Participate in a flexible and rotating on-call schedule, responding to emergencies and crises, providing backup coverage, and working nights and weekends as required.
  • Perform additional assigned duties.



  • A bachelor’s degree is required; a degree in psychology, special education, human services, or a related field is preferred.
  • One (1) year of experience working with individuals with Developmental Disabilities in a vocational, residential, case management or treatment program or equivalent preferred.
  • One (1) year experience working as an SSA for Hamilton County Developmental Disabilities Services.
  • Must pass background checks and driving abstract.

Compensation & Benefits

  • 36.25 work week, full-time role. Hourly position paid bi-weekly.
  • Flexible schedule and hybrid work-from-home options.
  • One week of paid vacation and 15 sick days immediately allocated upon hire. Three personal days are provided per year, with an employee start date. Starting vacation accrual of 3 weeks per year. Sick time accrued 15 days per year after the first year of employment with an unlimited accrual.
  • The starting salary range, determined by formula accounting for years of experience, is $50,895.00 (base rate) to $63,618.00 (10 years performing this job or a job with primarily the same duties).
