Early Intervention Service Coordinator
Job Description
A typical day in the life of an Early Intervention (EI) Service Coordinator involves coordinating the eligibility process and subsequently individualized family service plans for a caseload of families. You would regularly meet with or otherwise communicate with families to determine areas for developmental opportunity for their child and coordinate the funding, resources, and specialized services to be provided.

Job Duties (Summary):
- Assists parents of infants and toddlers with disabilities and/or developmental delays in obtaining access to needed early intervention services and other services identified in the individualized family service plan (IFSP), including making referrals to providers for needed services and coordinating appointments
- Coordinates the funding sources for early intervention services
- Coordinates and participates in evaluations and assessments as appropriate
- Facilitates the development of a transition plan to preschool or other services
- Advocates for children and families within the healthcare and educational systems
- Monitors and evaluates the progress of children and the effectiveness of intervention services
- Maintains records of the child, family and referrals in a confidential and up-to-date manner
- Completes data entry and other required early intervention forms necessary to document all service coordination activities
- Performs other related duties as required or assigned
Requirements
- Bachelor’s degree in special education, Early Childhood, or Social Work or related field
- Ability to travel within Hamilton County as needed
- Must be able to pass criminal background checks and various abuser registry checks
Work Location
This position follows a hybrid schedule, combining in office and community work. Employees are expected to travel within the county to meet with individuals and attend in-office meetings as needed. The flexibility to work from home is available when job duties allow.
