Spanish Speaking Developmental Disabilities Case Manager

Job Description

This position provides translation of materials and communication from English to Spanish and from Spanish to English, as needed, for a primarily Spanish-speaking caseload. In addition, the role serves as the primary service coordinator for support services.

A typical day in the life of a Service & Support Administrator (SSA) includes working in the field and meeting with individuals on an assigned caseload. Meetings may take place in an individual’s home, at a coffee shop, in one of the agency’s offices, or wherever the individual feels most comfortable.

The SSA conducts phone calls using a company-provided iPhone and participates in Zoom meetings with team members and service providers to coordinate and organize resources for individuals served. Responsibilities include entering case notes and updating planning documents using a company-provided laptop.

The role offers a blend of remote and field-based work. Part of the day may be spent organizing and communicating from home, while other portions involve traveling to meet with individuals. The SSA may also stop by agency offices to pick up supplies or work between appointments.

Job Duties (Summary):

  • Facilitates individual and family service plan development, assigning responsibilities among team members, and coordinating goal implementation.
  • Assists in identifying, selecting, and obtaining/maintaining services, support, and benefits (such as funding, housing, and educational services); serves as the primary contact for assigned individuals and families.
  • Ensures timely and accurate documentation adherence to regulatory requirements with supervisory support.
  • Monitors individual plans for compliance and service quality, communicating rights to individuals.
  • Prepares case notes, reports, evaluations, and summaries while maintaining confidentiality.
  • Collaborates with staff to problem-solve, identify resources, and address service gaps.
    Complies with relevant state and local guidelines for reporting abuse, neglect, and incidents.
  • Attends meetings and conferences for best practices, participating in relevant division, agency, local, and state meetings.
  • Translates English to Spanish and vice versa for required meetings, forms, and documentation.
  • Maintains required licenses and certifications.
  • Travels for service planning and monitoring in line with agency procedures.

 

Requirements (Education, Experience, Licensure, Certification):

  • Bachelor’s degree required, a degree in psychology, special education, human services, or related field preferred.
  • Ability to communicate (speak and write fluently) in Spanish and English is required.
  • One (1) year of experience working with individuals with developmental disabilities in a vocational, residential, case management, treatment program, or equivalent preferred.
  • Must be able to pass criminal background checks, driver’s abstract, and various abuser registry checks.

 

Unusual Working Conditions:

Service and Support Administrators are required to drive extensively within Hamilton County to perform their duties. Therefore, access to reliable transportation is a necessity and an essential function of this position. Planning meetings will be held in individual/family homes and throughout the community.

 

Start Date:

Hamilton County Developmental Disabilities Services hires Service and Support Administrators in groups. The next target date for onboarding will be April 23rd, 2026. If you are interviewed and considered for this position, you must be willing and able to begin working full-time for this agency on this date.
