Job Description

Hybrid – remote, in the community, and at HCDDS Kingsley Center: 5093 Kingsley Drive, Cincinnati, OH 45227

A typical day in the life of an SSA includes being out in the field meeting with individuals on your caseload. You might meet at their home, a coffee shop, one of our offices, or wherever the individual is most comfortable. You will also make phone calls from the iPhone we provide and attend Zoom meetings with others on your team or service providers to organize resources for your individuals served.

As Spanish Speaking Case Manager you will enter case notes and update individuals’ planning documents on the laptop we provide. Sometimes, you may organize and communicate from home for part of your day and out in the field, meeting with people in other parts of your day. You may stop into our offices to grab supplies or work for an hour between appointments. Your goal is to support your assigned individuals in living their best life.

 

Job Responsibilities

  • Carries a caseload of individuals whose primary language is Spanish.
  • Facilitates individual and family service plan development, assigning responsibilities among team members and coordinating goal implementation.
  • Assists in identifying, selecting, and obtaining/maintaining services, support, and benefits; serves as the primary contact for assigned individuals and families.
  • Ensures timely and accurate documentation adherence to regulatory requirements with supervisory support.
  • Monitors individual plans for compliance and service quality, communicating rights to individuals.
  • Prepares case notes, reports, evaluations, and summaries while maintaining confidentiality.
  • Collaborates with staff to problem-solve, identify resources, and address service gaps.
  • Complies with DODD and HCDDS guidelines for reporting abuse, neglect, and incidents.
  • Attends meetings and conferences for best practices, participating in relevant division, agency, local, and state meetings.
  • Maintains required licenses and certifications.
  • Travels for service planning and monitoring in line with agency procedures.
  • Works a flexible on-call schedule, responding to emergencies and providing backup coverage.

 

Qualifications

  • A bachelor’s degree is required, and a degree in psychology, special education, human services, or a related field is preferred.
  • One (1) year of experience working with individuals with Developmental Disabilities in a vocational, residential, case management or treatment program or equivalent preferred.
  • Must be able to pass criminal background checks and driving abstract check
  • Have a passion for improving others’ lives, demonstrated through superior customer service.
  • Possess excellent written and verbal communication skills and an appreciation for the work behind providing social services to the community.

Compensation & Benefits

  • 36.25-hour work week, full-time role. Hourly position, paid bi-weekly.
  • Flexible schedule and hybrid work-from-home options.
  • One week of paid vacation and 15 sick days are immediately allocated upon hire. 3 Personal days provided, prorated with an employee start date. Starting vacation accrual of 3 weeks per year.
  • Starting salary range: $47,125.00 (base rate) to $58,435.00 (determined by applicable years of experience).
